Surrey's Annual review forms for statements of special educational needs (SEN) have been changed. Following comments from schools and other colleagues, the form has been simplified and should now be easier to complete.

The new guidelines and the amended form are available from Surrey County Council's website. The revised Annual Review Summary Report Form (October 2011) replaces the forms and guidance previously provided.
