Person-centred transition reviews

With funding from the Learning Disability Partnership Board, Partnership with Parents has, since September 2007, been working with schools to introduce resources around person-centred transition reviews.

The first year of the project involved 20 of Surrey's special schools.  It was then extended, during the second year, to Surrey's mainstream secondary schools, and to those non-maintained and independent schools based in Surrey.  Information days were offered to schools and other professionals involved in Year 9 reviews.  46 of the 53 mainstream secondary schools attended!

A Resource Guide (shown above) was developed, with help from the Project Steering Groups as well as from the schools themselves - several special schools helped by trialling the resources before they were finalised.  The final guide was distributed to all the special schools and professionals concerned, and was handed out to participants at the information day held for the mainstream and NMI schools in April 2009.
